Position Classification: Program Specialist
Working Title: Intake/Case Manager
Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual's and families comprehensive needs through communication and available resources to promote self-sufficiency. This position is a renewable grant-funded position from January 2021 to December 2021. The work schedule is 30+ hours.
The position requires occasional fieldwork to inspect housing and to work in partnership with other organizations. You can find the complete job description here.
Duties include application screening, interviewing, explaining program requirements, consulting and collaborating with clients to find housing, facilitating rental transactions recommending the expenditure of grant funds, completing forms and reports, developing case files, participating in outreach activities, and entering information in various databases, particularly the Homeless Management Information System.
Qualifications include the ability to communicate effectively in writing and verbally, the ability to interpret complex regulations, experience interviewing clients and maintaining confidentiality, ability to establish and maintain inter agency partnerships, ability to create thorough and accurate case files, ability to multi-task, and ability to become proficient in a cloud-based information technology system.
